Position Available: Patient Care Navigator
Department: International Community Health Center
Location: Akron, Ohio
Schedule: 0.8 FTE or 32hrs/week, non-exempt position
Pay Rate: Based on qualifications

SUMMARY

Asian Services In Action, Inc. (ASIA, Inc.) seeks a highly motivated and bright individual who possesses a diversified set of skills for the Patient Care Navigator position at our International Community Health Center. The primary role of the Patient Care Navigator position is to focus on the patient needs and provide linguistic and culturally appropriate support with services offered at the health center. The Patient Care Navigator will assist and guide the patient through the health care system and overcome obstacles that are in the way of the patient receiving the care and treatment they require.

DUTIES AND RESPONSIBILITIES

  • Organize schedules and manage appointments for patients to ensure they receive services in a timely manner;
  • Facilitate communication between the patient, family members, and health care providers to ensure patient satisfaction and continuity of care;
  • Manage health care records;
  • Ability to navigate patient records, make appointments and referrals in EPIC;
  • Accommodate interpretive language services when needed;
  • Inform patients on financial aspects of care and sliding scale policy;
  • Manage follow up visits and ensure a continuation of care and linkage to community resources;
  • Adhere to ASIA-ICHCs client confidentiality policy;
  • Participate in trainings to include the health center electronic health records system (EPIC) for registration and patient navigation, and continuing education for medical terminology, health topics, etc.;
  • Assist the Refugee Health Screening Coordinator with non-medical activities of the Refugee Health Screening program at ASIA-ICHC;
  • Assist the Referral Coordinator when needed to contact patients;
  • Work occasional weekends and other weekday evenings for Community Health Promotions activities may be needed.

QUALIFICATIONS 

  • Bi-lingual skills are beneficial, Swahili preferred;
  • High School diploma or GED required;
  • Minimum of 1-2 years experience in health, social or community services or comparable;
  • Must be sensitive to the language and cultural needs of the patients served;
  • Must be adept in working with diverse staff who are multi-cultural and multi-lingual;
  • Requires some work on evenings and weekends;
  • Detailed oriented, organized, and ability to multi-task;
  • Excellent computer skills, including proficiency with MS Office and OS applications; 
  • Have access to the internet or computer at home;
  • Must pass a criminal background check 
  • Valid Ohio State drivers license preferred;
  • Must be authorized to work in the U.S.

The position reports to the Assistant Director of Operations, Refugee Health Screening Coordinator, and other

assigned supervisor as directed. We are seeking a candidate who has strong technical and soft skills, high energy, maturity and potential for leadership.
