Asian Services In Action’s International Community Health Center (ASIA-ICHC) is a federally qualified health center based in Midtown Cleveland’s AsiaTown.  ASIA-ICHC predominantly serves low-income, underserved, limited English-speaking Asian American and Pacific Islander (AAPI) and other immigrant and refugee populations of all ages.  Our mission is to empower AAPIs in Northeast Ohio to enhance their well-being, build their communities and to improve their self-sufficiency through culturally and linguistically relevant information and services.

ASIA-ICHC is looking for a Family Practice Physician to complement a small patient-centered care team.   The Physician provides primary health care to individuals and families in all aspects and developmental stages of life.  This includes the management of health problems and the promotion of optimal health. He or she reports to the Medical Director.  ASIA-ICHC is fully committed to the implementation of the Patient Centered Medical Home.

Duties and Responsibilities

A.  CLINICAL JUDGMENTS

  • Make sound clinical judgments based on assessment of the physical, psychological, spiritual, social and environmental needs of the client.
  • Develop a problem list, a diagnostic, therapeutic and teaching plan.
  • Determine which problems are within scope of practice and refer to specialty colleagues, including the interdisciplinary team, when necessary.
  • Analyze health behavior related to personality, life style, socio-economic status and culture.

B. CARE MANAGEMENT

  • Develop, implement, and periodically evaluate therapeutic plans.
  • Prescribe treatment for illnesses within his or her scope of practice.
  • Order and interpret diagnostic studies.
  • Teach and counsel individuals and families to assist them to assume responsibility for the prevention of illness and restoration and maintenance of health.
  • Evaluate and monitor patient’s physical and emotional reaction to drugs and treatments.

C.  PROFESSIONAL RELATIONSHIPS

  • Communicate, consult and collaborate with other professionals involved in the delivery of total patient care.  ASIA-ICHC has Memos of Agreement with several community hospital and specialty service systems including MetroHealth Medical Center, St. Vincent Charity Hospital, and NEON and Care Alliance for dental access.
  • For those interested in teaching and academic development – there are opportunities to become a clinical faculty member of Case Western University School of Medicine and/or Northeast Ohio Medical University.  There will be opportunities to work with both medical students and residents if interested.  There will also be opportunities to participate in practice-based research networks, program and policy development.
  • Keep up to date complete problem-oriented records and participate in collaborative team-based care.
  • Be available to participate with community agencies to coordinate comprehensive care for patients.
  • Attends meetings, events and activities as expected of other ASIA-ICHC staff.
  • Maintain CME and participate in other pertinent job training sessions related to the clinical practice.
  • Perform other related duties as assigned

Qualifications

  • Specific knowledge, training or skills required to perform the duties of this position.
  • To perform this position successfully, an individual must be able to perform each essential duty satisfactorily.   • Must be a graduate of an accredited medical school.
  • Current Ohio license to practice medicine (MD or DO); must be Board certified or provide letter indicating qualification to sit for certification with expectation of obtaining certification within one year.
  • Minimum two years’ experience as a MD/DO in community medicine preferred.
  • Computer proficiency: ability to utilize an electronic medical record and email.
  • Ability to communicate in English, bilingual a plus, but not required. There will be patients who speak Chinese dialects, Arabic, Nepali, and Karen/Burmese.  Cultural competence and general interest in the rich diversity of patients is more important than language proficiency.
  • Demonstrated clinical competence, including physical assessment, health teaching and counseling.
  • Emphasize commitment to continuing professional development through independent reading and study, and through participation in continuing education programs.
  • Physician can perform minor diagnostic and surgical procedures within scope of practice.
  • Physician may be in a collaborative relationship with others in the practice and will be expected to participate in some supervisory level care when needed.
  • The physician will follow and may participate in development of performance improvement procedures and quality control for ICHC.
  • Passionate about ASIA’s mission and impact
  • Proficiency with EPIC EMR/PMS system or other comparable system
  • Access to transportation with valid driver’s license required
  • Authorized to work and pass criminal background check

General Information

  • Prescribe treatment for illnesses within the Family Nurse Practitioner’s scope of practice.

  • Teach and counsel individuals and families to assist them to assume responsibility for the prevention of illness and restoration and maintenance of health, use of prescribed medical treatments and/or medications.
  • Monitor and evaluate patient’s physical and emotional reaction to drugs and treatments.

  • Must hold current Ohio Registered Nurse licensure, Certificate of Authority and Certificate to Prescribe.
  • Must be a certified FNP with MSN or Doctoral degree..
  • Minimum two years’ experience as a Family Nurse Practitioner – preferred
